Beneficiary Notification of Payer/Insurance Participation:
According to Public Act 93-0261 in the Illinois Insurance Code, “when a person presents a benefits information card, a health care…provider shall make a good faith effort to inform the person if the healthcare provider has a participating contract with the insurer, health maintenance organization, or other entity identified on the card.”

If the payer identified on your insurance card is NOT listed above, OSF Saint Anthony Medical Center does not have a participation agreement with that particular insurer/payer. Be aware that when you elect to utilize the services of a non-participating provider for anything other than emergency services or services approved by the payer/insurer in advance, the basis of your benefit payment will be determined according to your insurance policy’s “out-of-network” fee schedule and you will be responsible for the balance. Some insurers have adjusted their out-of-network fee schedules to a very low rate. If you receive services here, you are STRONGLY encouraged to obtain information on out-of-network expenses by calling the telephone number on your insurance identification card.
